Enrollment Forms
Enrollment Procedures include:
- Complete & return the
Application Formwith $50 registration fee per student (non-refundable) - Interview between parents and principal, teacher, pastor or School Board member
- Child’s completion of placement test(s), if necessary
- Approval of School Board
- Completion of medical emergency consent and health form
- Receipt of records from previous school (if any)
- Receipt of immunization records; all immunizations up-to-date
- Receipt of signed tuition agreement and first tuition payment
