Enrollment Forms

Enrollment Procedures include:

  • Complete & return the Application Formwith $50 registration fee per student (non-refundable)
  • Interview between parents and principal, teacher, pastor or School Board member
  • Child’s completion of placement test(s), if necessary
  • Approval of School Board
  • Completion of medical emergency consent and health form
  • Receipt of records from previous school (if any)
  • Receipt of immunization records; all immunizations up-to-date
  • Receipt of signed tuition agreement and first tuition payment
